Dear Patients of One Fertility Kitchener Waterloo,
We are happy to announce that we will be resuming fertility testing and treatments from the 19th of May 2020. Our center will be opening with necessary precautions and measures in place for the safety of our patients and staff. It is important for everyone to abide by these precautions and measures prior to and during visits to our center.
- All visits to the clinic will strictly be by appointment only. Please telephone the reception to book appointments as required.
- Patients will be required to sign a COVID-19 waiver prior to their appointment.
- We will reduce the number of visits to the clinic by offering appointments only as necessary. Any other appointments will be virtual through Ontario Telemedicine Network (OTN).
- For ALL visits to the clinic you will need to arrive wearing a mask. Please ensure that you have a mask for your scheduled appointment.
- For in-clinic appointments, patients are to arrive 5 minutes prior to the scheduled appointment time. You are to remain in your vehicle. A staff member will call you when you are to enter the clinic. You will be prompted to complete mandatory screening prior to entering the clinic. This will include temperature (noncontact) and screening questionnaire. We will bring you into the clinic when this is completed.
- Our staff also completes mandatory screening questionnaires prior to starting their daily shifts.
- Patients who are coming for testing or monitoring who require ultrasound and/or blood work will do ultrasound and/or blood work and proceed to leave the clinic once finished. You will not see the nurse after your appointment. You will be contacted later that day to discuss results and next steps via Telemedicine by either the fertility nurse or our reproductive endocrinologist
- Patients who may need medications should ensure that they have the necessary medications before leaving. If you are not sure if you need medications, you will be advised in the Telemedicine consult later that day.
- Please ensure that you remain 6 feet apart from other individuals while in the clinic. In certain locations in the clinic there are taped lines on the floor which clearly designate 6 feet physical distancing.
- If you have ANY symptoms, PLEASE report these to one of our staff members for further instruction and refrain from coming into the clinic until further instruction is given.
- Staff will be ensuring to maintain the standards of practice when sanitizing and disinfecting all surfaces. We will also be limiting patient interaction with any unnecessary areas in the clinic.
- Staff will be wearing appropriate PPE when interacting with any patients as needed.
